Improving Health Outcomes by Engaging the Whole Community

Initium Health is adept at conducting Community Health Assessments (also known as Community Health Needs Assessments) and crafting Community Health Improvement Plans. We understand the importance of these assessments to organizations and the communities they serve, so we foster an inclusive process to include both major partners and people whose voices are not always heard.

Utilizing interpreters and our own diverse and multilingual staff, we approach all groups with cultural sensitivity. We use the established MAPP 2.0 process by the National Association of County and City Health Officials (NACCHO), which gives breadth and credibility to our health assessment and improvement plans. The MAPP 2.0 process drives a series of collaborative community assessments that guide our research, community engagement, and document creation.

If you are with a Health Department who is seeking Public Health Accreditation Board (PHAB) certification, we understand the requirements. If you are with a healthcare organization needing a new community health assessment to make a case with stakeholders, we understand that also. Our ongoing process of Research, Outreach, and Marketing means that we are sure to reach your target population in a meaningful, respectful, and engaging way. 

Our Outreach team helps to identify and amplify community members’ concerns, and to collaborate with community partners or coalitions who may be suited to help address those concerns. Our Research team helps to identify the strengths and opportunities in any community, generating qualitative and quantitative research reports, with a lens focused on the community’s most vulnerable members. Our Marketing team creates outreach materials that drive community engagement, measuring analytics on an ongoing basis to ensure we are truly reaching the correct groups with the correct message. And because we have team members with backgrounds in design and advertising, we collaborate with clients to create deliverables that are visually stunning and have a local feel.

Lubbock Public Health

From September 2023 to June 2024, the Lubbock Public Health Department collaborated with Initium Health of Denver, Colorado, to conduct a Community Health Needs Assessment (CHNA) in Lubbock County, Texas.

The Initium Health team was hired to evaluate emerging data and local health needs, then outline strategic initiatives to enhance community well-being in Lubbock and the surrounding areas.

Hays County

Situated in central Texas, Hays County is a diverse community characterized by a blend of rural and suburban regions. In recent years, the county has witnessed substantial population growth.

The multicultural population encompasses various ethnicities, prominently including Hispanic, White, and African American residents. People of diverse age groups, ranging from young families to retirees, reside in the county, with a rising number of college students attending institutions like Texas State University.

Notably, this population expansion has also brought about disparities in access to healthcare, human services, and other essential resources.
